Appointments:
We currently welcome new patients. Same-day appointments are available.
Insurance:
We participate in most insurance plans, including Medicare, No Fault (auto accident) and Workers Compensation. We do not accept Medicaid.
Please call the office if you have any questions regarding your insurance coverage. We make every effort to maximize your insurance benefits.
Co- payments and deductibles:
All co-payments and deductibles must be paid at the time of service.
Payments can be made in the form of cash, personal check or credit card. We accept MasterCard, Visa, and Discover.
Coverage Changes:
If your insurance changes, kindly notify us as soon as possible. This will allow us to ensure your maximum benefits. If there are any questions, kindly do not hesitate to call our office.
New Patient Paperwork
For your convenience, you may complete new patient paperwork in advance. Please call the office to arrange pick up.
If you are unable to complete the forms before your visit, please arrive 30 minutes early for their completion. If you need additional assistance, help will be provided.
You will need to:
Bring photo ID
Bring your insurance card.
Bring list of current medications and prior surgeries.
Bring a copy of any current records you may possess (i.e. X-Ray or MRI reports or discs, other relevant information) if possible.
Note that if a co-pay is required by your insurance company, we are required by law to collect it at the time of your visit.
Your first visit
Upon arrival, you will be greeted by our friendly staff. We will supply you with a “New Patient” packet. Kindly bring your photo ID and insurance cards. We are required to make a copy for your file.
Our new patients will view a short video discussing chiropractic care. Upon completion of the video, the doctor will present him or herself and review your medical history. The doctor will then perform an orthopedic/neurological examination. An examination is vitally important for determining the proper course of treatment. If immediate treatment is required, it will be addressed at this time.
After the doctors’ consultations and examination, they will discuss any additional testing that may be required. In most cases imaging (ie. X-Ray or MRI) will be necessary. If your condition requires medical intervention, we will refer you to the appropriate specialist in conjunction with chiropractic care.
After a review of the history, examination findings, and imaging, the doctor will determine the required treatment schedule. Note: this is most often determined on your second visit. It is crucial that you adhere to your treatment schedule, to maximize a successful outcome.
You may also be provided with home instructions to assist with any discomfort. These may include ice/heat recommendations, supports, home exercises, etc.
